Every 8 hours medication schedule

Every 8 hours means three doses with even gaps, such as 6 AM, 2 PM and 10 PM. Start at 6 AM and all three fit before bed, and the overnight stretch from 10 PM to 6 AM is 8 hours too. Start at 8 AM, the way the two hospital pharmacies we checked do, and the third dose lands at midnight. The schedule maker below is set up for every 8 hours.

Medications

Add each medication, the dose, and how often it's taken. The printout puts each scheduled dose in a column at the time it's due.

Leave it empty and you'll get a blank schedule grid to fill in by hand. The times are sensible defaults you can adjust on the printout. We generate the PDF and send it back — we don't save it, log it, or keep anything you type.

The printout puts every-8-hours doses at 6 AM, 2 PM and 10 PM. If you get up later, move all three by the same amount (7 AM, 3 PM, 11 PM) so the gaps stay 8 hours.

Three doses 8 hours apart cover 16 hours from the first to the last. That's nearly the whole 17 hours between 6 AM and 11 PM, so the time of the first dose decides whether anyone sets an alarm. Both hospital tables start at 8 AM, which is the whole reason their third dose is at midnight.

UF Health's pharmacy bulletin says "3 times a day" is not the same as "every 8 hours." For oral medicines, every-8-hour dosing means waking the patient, which may or may not be necessary depending on the medicine. That's the question to take home from the hospital: does this one need the midnight dose, or can the times move?

Add 8 hours, then 8 more. Any dose that lands in the example night (11 PM to 6 AM) is marked:

Does every 8 hours mean waking up at night?

Often not. In their guide to giving a child medicine, AAFP's family doctors say "take every 8 hours" generally means three times a day, and Medical News Today's pharmacists say most medicines are taken during waking hours.

Some medicines are different. UConn's pharmacy lists antibiotics, pain medicines and seizure medicines as the ones that work better at evenly spaced intervals, to keep a steady level in the blood. Medical News Today notes that some antibiotics may need overnight doses. For those, even spacing is the instruction, and an early first dose is the simplest way to keep it without an alarm at 2 AM.

Every 8 hours next to the rest of the day

Different wording for the same number of doses is where home schedules split. In Wolf and colleagues' 2011 study, when two medicines said the same thing two ways ("every 12 hours" and "twice daily"), 79% of the people in the study didn't take them together.

This chart puts the every-8-hours times right next to the morning, noon, evening and bedtime columns that the day's other medicines use. You can see on one page where the even doses fall among everything else.

Questions about dosing every 8 hours

What times are every 8 hours?
Any three times 8 hours apart: 6 AM, 2 PM and 10 PM; 7 AM, 3 PM and 11 PM; or 8 AM, 4 PM and midnight. Pick the set whose last dose comes before you go to sleep.
Is every 8 hours the same as 3 times a day?
Both are three doses, but they aren't the same instruction. Three times a day is usually spread across waking hours. Every 8 hours asks for even gaps, and both hospital tables we checked put the third dose at midnight. For a child's medicine, though, AAFP says every 8 hours generally means three times a day. Ask the pharmacist if this medicine needs the even spacing.
What does q8h mean?
q8h means every 8 hours. Like q6h and q12h, it's an abbreviation you'll mostly see on prescriptions and hospital papers. The pharmacist writes it out in plain words on the bottle.
